When a client needs a home health care provider we proceed as follows:
- Family Meeting: to discuss the specific and individual needs of their loved one which includes not only their medical and personal care but also their emotional care. Especially when dealing with a patient who has Alzheimer’s or Dementia, it is exceptionally important that we are ultra-sensitive to the sort of personality of the caregiver who will best be able to relate and handle this special needs client.
- In-Home Assessment: one or more of our staff members will come to do an in-home assessment of the residence to determine if all necessary safety/physical assistance aides are in place for the safety of your loved one. This will include items such as, but not limited to: ramps, grab bars, chair lifts, medical equipment (walker, cane, shower chair, lift-assisted recliner) & or hospital bed. If there is oxygen necessary in the home this will be addressed as well.
