  1. Free Consultation to review your case and advise whether you currently qualify for Medicaid either for Long Term Community Health Care (in your home) or Nursing Home Care (in a skilled Nursing Home facility).
  2. Assistance in identifying, procuring and preparing the documentation you will need to supply to Medicaid for your application which will be prepared either by us and or an attorney we have recommended who is preparing same.
  3. Referring you to one of our Strategic Partner Attorneys or assisting navigating the relationship with your legal team during the process so as to reduce time expended and keep costs down if you cannot get a flat rate for their services.

  5. Enrolling with a Pooled Income Trust and teaching you how to use it (to address excess income that you receive over the allowed limit by Medicaid and how to shelter that income rather than pay it to Medicaid)
  6. Assistance responding to “Pend Letters” from Medicaid which are requests for additional information during the application review process.
  7. Finding and dealing with the Managed Long-Term Care Company (MLTC) enrollment process including what to say and do during their initial “Conflict Free Evaluations” to determine necessary care/hours for your loved one.
  8. Identifying and engaging a Home Health Care Agency in your area to provide multiple aides for your loved one.
  9. Identifying & enrolling with the proper Managed Long Term Care Company that will oversee the process and ultimately approve hours for the home health care provider(s) for your loved one.
  10. In-home evaluation of residence for living conditions & assurance of safety for your loved one.  Assessment of loved one’s physical care needs and recommendations for the necessary level of care.
  11. Assistance with the CDPAP enrollment process where you are able to have a family member, friend or person of your choosing to become your loved one’s caregiver and be paid by Medicaid for such services.
  12. Assistance with the annual recertification process to maintain your Medicaid status

  14.  Assisting with what happens once your loved one has permanently moved to a nursing home or has passed away: Funeral Arrangements, preparing to sell the home, appraising the home, clean out’s, tag & estate sale, renovating the property to ensure the best sale price, appraising the residence and engaging the best real estate professional based on the property location and value.
